1. Medical Information

The content on this Site, including answers from our chat assistant, is general information and is not medical advice, diagnosis or treatment. Using the Site does not create a doctor-patient relationship. Always ask your eye doctor about your own eyes and health.

If you have a medical emergency, call 911. For an urgent eye problem, call our office at (516) 266-9398. Do not use our forms, chat or text messages for emergencies.

2. Appointment Requests and Online Forms

A request sent through this Site is not a confirmed appointment. Our office will contact you to confirm a time.

You agree to give accurate and complete information in our forms. If you send a form for someone else, such as your child or a patient you are referring, you confirm that you are allowed to share their information.

3. Text Messages

Citywide Eye Care sends text messages to patients who agree to receive them. We do not send marketing texts. We send:

• Appointment reminders and confirmations

• Replies to messages you send us

• A note when we miss your call

You agree to receive texts by checking the text message box on our appointment request, contact or doctor referral form. By agreeing, you confirm that you are the account holder for the mobile number or have the account holder's permission. Agreeing is not required to book an appointment or receive care.

• Frequency: Message frequency varies.

• Cost: Message and data rates may apply.

• Stop: Reply STOP to any text to stop receiving texts. Reply START to receive them again.

• New number: If you change or give up your mobile number, please let us know so we stop texting it.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third parties. See our Privacy Policy.

4. Emails

If you subscribe to our newsletter, we may email you about eye health, our services and promotions. Every marketing email includes an unsubscribe link.

5. Insurance, Prices and Promotions

Insurance, product and service information on the Site is general and may change. Your coverage depends on your plan and is confirmed by our office. Promotions may have their own conditions and may change or end at any time. Refunds follow our Refund Policy.

6. Privacy

Our Privacy Policy explains how we handle information you give us through the Site. Health information we keep as your eye care provider is handled as described in our Notice of Privacy Practices.

7. Using This Site

You agree not to:

• Use the Site for anything unlawful

• Send false information, or information about someone without their permission

• Try to get into parts of the Site or our systems you are not allowed to use

• Interfere with the Site, including by sending viruses or automated traffic

8. Our Content

The text, images, logos and design of the Site belong to Citywide Eye Care or are used with permission. You may view and print pages for your own personal use. Do not copy, change or reuse them for any other purpose without our written permission. Product names and logos, such as those of contact lens makers, belong to their owners.

9. Links to Other Websites

The Site links to other websites, such as maps, social media and product makers. We do not control those websites and are not responsible for their content or privacy practices.

10. Disclaimer

The Site is provided "as is" and "as available." We work to keep it accurate and available, but we do not promise that it will always be accurate, complete, uninterrupted or free of errors. To the extent allowed by law, we make no warranties about the Site, express or implied.

11. Limitation of Liability

To the extent allowed by law, Citywide Eye Care is not liable for any indirect, incidental or consequential damages that come from your use of the Site. Nothing in these Terms limits liability that cannot be limited by law.

12. Indemnification

You agree to cover any claims, losses or costs, including reasonable legal fees, that come from your misuse of the Site or your breaking these Terms.

13. Governing Law

These Terms are governed by the laws of the State of New York. If any part of these Terms cannot be enforced, the rest still applies.

14. Changes to These Terms

We may update these Terms. The date below shows when they last changed. Using the Site after a change means you accept the updated Terms.
